Medical Release Form

Medical releases for attending students (ages 18 & under) are mandatory, even if parents are staying on campus.

Please be aware that the Summer School Committee is not able to allow any exceptions.

A blank copy of the Medical Release form is included with the Registration Form.

When applicable, please complete, sign and return to the registrar.
